Registration Information

There are several documents Norwich Free Academy requires prior to enrolling a student. You may download the pre-registration packet for Norwich residents by clicking here, non-Norwich residents by clicking here. You may also pick up your appropriate packet at the Allis House (click here for campus map).

All forms in packet must be filled out and signed
Proof of residency (lease agreement, mortgage documents or utility bill) A drivers license is not acceptable
Student’s birth certificate (long form)
Student’s social security card
Student’s most recent transcript and all other prior grades
In the case of a change of child custody, court documents or a notarized letter from the custodial parent
Student’s immunization record and last physical

IMPORTANT: Residency Affidavit, which has been completed and signed by your town’s Superintendent of Schools. Take your proof of residency (lease agreement, mortgage documents or utility bill) with you for approval
