Students with Diabetes should provide the following items to the school nurse - Authorization forms for blood glucose test and glucagon from physician to include may test at own site on campus
- Glucose meter with extra batteries
- Test strips for the meter
- Lancets to prick finger
- Urine ketones strips
- Glucagon emergency kit
- Glucose Gel
- Glucose tablets
- Juice boxes (4 oz.) to treat low blood sugars
- Extra snacks (Please be sure your child also keeps a snack in backpack.)
- Pump supplies (if using pump), extra insulin, infusion set, extra batteries
NOTES:
A nurse is not present on field trips, parent may attend
