Connecticut State Law requires all new entries and re-entries to be completely immunized before they start school. Requirements are: - Measles
- Two doses at least one month apart.
- First dose after first birthday
- Mumps
- One dose given after first birthday
- Rubella
- One dose given after first birthday
- DPT
- Four doses - last dose after fourth birthday
- Polio
- Four doses - last dose after fourth birthday
- Proof of Varicella (chickenpox) disease or date of Varicella immunizations
- One dose if given before the age of 13
- Two doses one month apart if given after 13th birthday
- Hepatitis B
- Results of tuberculosis risk assessment
- If considered high risk for tuberculosis, tuberculin skin test must be performed, interpreted and managed as per the Tuberculosis Control Screen Guidelines for Connecticut Schools.
- A physical exam performed within the past year
- A copy of the enrolling student's latest physical and immunization record is needed prior to school entrance.
Should the student transfer from The Norwich Free Academy to another school within Connecticut his/her cumulative health record will be sent to the new school. If the student transfers out of state the Medical Center will provide the parent/guardian with a copy of his/her latest physical exam and a copy of immunizations upon his/her request.
