Students should report all injuries occurring during the school day immediately to the school nurse, or as soon as possible following an after-school activity. Any medical documentation received from the licensed care provider should then be provided directly to the school nurse.
Procedure for Sports Injury
In the event of an injury occurring as a result of a Norwich Free Academy sport's activity it is the responsibility of an injured student's parent/guardian to complete the Notification of Injury Report and return it to the Medical Center within one week of the date of the injury.
This report will be reviewed by the responsible coach and sent by the School Nurse to the Academy's insurance carrier.
Basic student insurance, for which parents pay the premiums, does not cover interscholastic sports claims, but the Academy carries and pays for separate insurance to help pay for sports related medical expenses in excess of any other coverage. There should be no direct cost to you.
First, interscholastic sports claims must be presented to your own insurance carrier, (Blue Cross, Blue Shield, or other medical prepayment plans). If both parents have coverage it must be submitted to each plan. After the claim is partly paid by these insurances, the remaining bills, together with the explanation of benefits letter which state the amount paid by the original carrier should be sent by the Parent/Guardian to:

Bailey Insurance will make payments directly to the Provider of Service, provided they are in receipt of the original Notification of Injury Report and treatment was sought within 90 days after the injury occurred.
